Hours of operation vary at each of the four Millennium locations. Please call the office you are interested in scheduling with and one of our staff members will assist you in finding a convenient time.

Insurance Concerns

Millennium Physical Therapy is credentialed by Medicare and several commercial insurance carriers. Millennium is also a provider of auto and workers compensation cases.

We recognize that today’s insurance environment is often complicated and confusing for patients and their families. We work hand in hand with our patients to make the insurance process as seamless as possible.

Patient Privacy

Millennium recognizes the importance of maintaining patient privacy. As per HIPAA guidelines we maintain a Notice of Privacy Practices that is posted and readily available in all our office locations. If you have a question or concern regarding patient privacy please address them to your therapist of record at any of our three locations.
